I hereby acknowledge that participation in Camp Korea and camp-related activities is at the sole discretion and judgment of the parent or guardian and involves an inherent risk of personal injury.

I, on behalf of my son/daughter, hereby assume all such risk. I hereby release and agree to hold harmless Camp Korea, C3 Korean Canadian Society (“C3 Society”) and its volunteers from all claims, actions, damages and liabilities for personal injury or damage relating to or arising out of any camp activity except where the injury or damage is caused by the gross negligence of the camp's volunteers.

In allowing my child to attend Camp Korea, I, the undersigned, permit my child to participate in the full range of camp activities and authorize the Camp Director or his/her appointee, in the event of a serious accident or illness affecting this above named camper to authorize on my behalf all procedures, including admission to hospital and necessary treatment therein, as he/she may deem essential for the care and well-being of the camper. Such action is only to be taken when immediate contact with the undersigned cannot be made. It is understood that Camp Korea is not responsible for medical care costs.

By checking this box, I confirm that I am the legal guardian of the registrant and that the registrant has received a full series of COVID-19 vaccines. Proof of vaccination will be provided upon request.

By checking this box, I confirm that I am the legal guardian of the registrant and that the registrant does not have known communicable diseases or health concerns (such as fever, chicken pox, lice, viral infections, rashes, athlete's foot, impetigo, symptoms of COVID-19, etc). If the registrant has been exposed to anything communicable within two weeks prior to the program, I will contact Camp Korea committee prior to the registrant's arrival.

I hereby grant permission to C3 Korean Canadian Society (“C3 Society”) the right to reproduce, use, exhibit, display, broadcast, distribute and create derivative works of the images of me taken through video, audio or photography during Camp Korea (event) for use in connection with the activities of C3 Society or for promoting, publicizing or explaining C3 Society or its activities.

This includes, without limitation, the right to publish such images, video and audio on the C3 Society website, for use in PR/promotional materials, such as marketing publications, newsletters, fundraising materials and other C3 Society- related publications.

These images may appear in any of the wide variety of formats and media now available to C3 Society and that may be available in the future, including but not limited to print, broadcast, videotape, CD-ROM and electronic/ online media. Individuals over the age of 18 may sign for themselves; individuals under 18 must have this release signed by a parent or guardian.
